Face to Face Letter | Attendance Verification Protocol
Dear Parents/Guardians:
Due to our current circumstances, we are requiring that all parents make an appointment with Ms. Judy Goris (718) 696-3800 extension 4210 to pick up any attendance verification letter. This includes letters for:
- IRS
- HRA
- Housing
- Bank, etc.
ONLY PARENTS/GUARDIANS CAN PICK UP THIS FORM. ALL PARENTS/GUARDIANS MUST PROVIDE A VALID ID THAT MATCHES OUR RECORDS SYSTEM. THIS IS REQUIRED BY THE NYCDOE.
VALID IDs ARE ONE OF THE FOLLOWING:
- NYS LICENSE
- NYS ID
- PASSPORT
- GREEN CARD
Please note: anyone that enters the building MUST FOLLOW ALL SCHOOL SAFETY PROTOCOLS.
YOU WILL BE REQUIRED TO HAVE YOUR TEMPERATURE TAKEN UPON ENTERING THE BUILDING AND WEAR A MASK.
